    Abstract
This dataset comes from Mzantsi Wakho — a longitudinal, mixed-methods community study of adolescents living with HIV in the Eastern Cape Province, South Africa, a region marked by structural vulnerability. The study aims to identify risk and resilience factors that influence antiretroviral therapy (ART) adherence; access to sexual and reproductive health (SRH) services; and other HIV-care outcomes among HIV-positive adolescents. Through linked quantitative and qualitative components, the project engages HIV-positive youths, their caregivers, healthcare providers, and social-service actors to better understand barriers and facilitators to effective HIV care and SRH service uptake. Data were collected over three survey waves (Wave 1: 2014–2015; Wave 2: 2016–2017; Wave 3: 2017–2018), combining self-reported questionnaires, clinic record abstraction, and facility-level service profiling — allowing analysis of temporal trends in resilience, treatment adherence, and service utilization. However, the data shared here relates to the self-reported questionnaires only. This study contains data from 1 563 adolescents (1 046 living with HIV at baseline).
